  • Are you a Stanford employee or the spouse/partner of an employee?

  • Are you a non-contingent, benefits eligible employee at Stanford University? (Not post-doc, hospital staff or retiree.)

  • Will you take your SHALA (Stanford Health and Lifestyle Assessment) this year?

  • Completing SHALA unlocks the Wellness Profile and all of its additional incentives, plus discounted fitness classes and free select sporting tickets.

    Are you enrolled in a university medical plan for 2018 (ex: Kaiser, Blue Shield, Aetna or Stanford Health Care Alliance)?

  • Did you agree to share your 2017 data?

  • You are eligible for $200 for completion of the 2017 Wellness Profile, (steps 1-3). This incentive is paid out two pay periods after completion of the Plan.

    Will you also complete the 4th step, Engagement?

  • You are eligible for $200 for completing the Wellness Profile.  This incentive will be paid out two pay periods after your Plan is complete.

    Additional Incentive! Will you complete six berries or participate in your Healthy Work Environment?

  • Are you enrolled on your Stanford employee’s medical plan?

    Are you also sharing your data?

  • In this case, you are eligible to participate in Screening and Advising for your own personal goals, but are not eligible for financial incentives.

  • You are eligible for $240 for completion of the 2017 Wellness Profile (steps 1-3). It is paid out in a lump sum in February 2018.

    NOTE: your spouse (the Stanford employee) must also complete the Wellness Profile (steps 1-4) for you to receive the incentive.

  • If you are both an employee and a spouse/partner of another Stanford employee, please contact bewell@stanford.edu to determine your eligibility.

  • You are unfortunately not eligible for the BeWell program. Please note Hospital employees can participate in Healthy Steps. Post-docs at the University are not eligible.

  • You are eligible for $100 for participating in a Healthy Work Environment.

    SHALA is not a prerequisite to this program.

  • You are eligible for $200 for the completion of the 2017 Wellness Profile (steps 1-3).

    You may complete the Engagement step of the Wellness Profile, but it is not required to receive your $200. It will be paid out in two pay periods after your Plan is completed.

    Additional Incentive! Will you complete six berries or participate in your Healthy Work Environment?

  • You are eligible to receive $280 in February 2018 for completing Step 4, Engagement.

    Additional Incentive! Will you complete six berries or participate in your Healthy Work Environment?

  • You are not eligible for an additional financial incentive.

  • On top of your Wellness Profile Incentive, you are eligible for an additional $100, paid out two pay periods after completing Berries or Healthy Work Environment.
