Registration is starting for the Spring/Summer season.
Here's what you can do to help out the team manager:
If you were registered for the Fall 2004 season:
- Tell me that you want to play ... email, telephone, carrier pigeon,
whatever works for you.
- Download and print a copy of the Player Registration Form. You may
choose either the Microsoft Word or
Adobe Acrobat version (the Microsoft Word
file may need to be repaginated).
- Fill out both pages of the Player Registration Form.
- Photocopy some proof of medical insurance.
- Fax or email the signed
Player Registration Form and proof of medical insurance to me.
- The per player registration fee is $65 this season. You can
mail a check
to me (my address is at the bottom of the page), or pay me in person. (I do have a PayPal
account, if that's your your preference.

- I'll need two photographs of your face for your player pass. Passport photos work, but so do
other photos with your face in them. The photos do not need to be identical. I
can print from digital files if that is easiest for you.

signed Player Registration Form, photocopied proof of medical insurance, and photos of you to
me.
