Surgery is the most radical treatment available for CTS. In carpal tunnel release surgery, an incision is made in the tranverse carpal ligament. While any incisions in the skin are sewed up, the incision in the ligament is left open. This relieves pressure on the median nerve. The carpals are bound together tightly enough to prevent most decreases in structural integrity of the wrist as a result of the incision in the ligament. Scar tissue forms in the ligament incision, resulting in an enlarged carpal tunnel. Carpal tunnel release surgery provides almost complete relief from all symptoms in 95% of patients.
