Currently, two types of surgery are practiced. In open incision carpal tunnel release, an incision is made through the transverse carpal ligament directly through the palm and intervening tissues. In endoscopic carpal tunnel release, a small incision is made below the wrist and a blade is inserted through a tube placed through the carpal tunnel. This tiny blade is used cut the transverse carpal ligament.

Both surgeries are minor and performed on an out-patient basis. While endoscopic surgery is less invasive, allowing reduced discomfort and faster healing, it is more technically demanding and presently provides greater risk of damage to nerve tissue. Being a relatively new procedure, this risk may decrease as techniques and technologies improve. For now, the debate still stands as to which procedure is safer and to a greater extent side-effect-free.
